The Department of Health Care Services (DHCS) developed the Social Worker and Case Manager Resource Guide to support social workers and case managers as well as other health advocates and community organizations that engage directly with dual eligible beneficiaries.
This reader-friendly resource guide provides information for social workers and county case managers about the basics of Cal MediConnect health plans, helps identify their clients’ options, and provides guidance in coordinating their clients’ care. The resource guide was developed with questions identified and complied by our on-the-ground outreach team.
Robust outreach and education is a core part of the design and implementation of the Coordinated Care Initiative and Cal MediConnect. DHCS is committed to assess and develop new tools to use in outreach and education activities.
