Today, we are pleased to announce that DHCS submitted an amendment to the Special Terms and Conditions (STC) and Expenditure Authority of Waiver 11-W-00193/9, California Section 1115 “Bridge to Reform” Demonstration (Waiver).
The amendment would allow the Department of Health Care Services (DHCS) to carry out the State of California’s Coordinated Care Initiative (CCI) in eight select counties, in order to: integrate Medicare and Medicaid benefits for dual eligibles, mandatorily enroll dual eligibles, and integrate long term services and supports (LTSS) as managed care benefits.
DHCS has requested that this waiver amendment have an effective date of January 1, 2014 and is working diligently with the Centers for Medicare and Medicaid Services (CMS) in order to secure prompt approval of this amendment.

I am concerned that enough patient’s rights are not incorporated into the link for “N” to ensure that home and community-based services are mandated rather than the above “transition” facility biased…a return to old ways when “REFORM” is the intention, of is it? A reminder that any/all printed material given to a patient to read or sign must be in large print and other requested alternative formats to printed materail. If alternative formats are required then this should be put into the patient’s file in large letters in a chosen color to bring attention to it on the patient’s cover both in a medical facility and a braclet indicating large print for the smallest matters like menues to medications, patient instructions for discharge, any literature the discharge planner may have for the patient. There is no direction to the discharge palnner and what they must do in all 8 demonstration counties. Maggie Dee-Dowling
