Revised Enrollment Chart by County Available Here!

On March 25th, 2014, the Department of Health Care Service (DHCS) announced a change to its Coordinated Care Initiative (CCI) enrollment timeline and strategy in order to continue to move forward to provide high quality, coordinated care to beneficiaries eligible for Medicare and Medi-Cal.  In an effort to keep interested parties informed and apprised of the latest developments of the Coordinated Care Initiative and to assist in the understanding of the recent changes, on Friday, 3/31/14, DHCS released the revised enrollment chart attached below.
