Cal MediConnect Health Risk Assessment (HRA) Data Now Available

Today, the Department of Health Care Services (DHCS) is releasing new data on Health Risk Assessment (HRA) completion rates in the Cal MediConnect program. HRAs are a critical part of the Cal MediConnect care coordination process.

Cal MediConnect HRAs
The Cal MediConnect program aims to coordinate medical, behavioral health, long-term institutional and home- and community-based services through a single health plan for beneficiaries dually eligible for both Medicare and Medi-Cal. That care coordination starts with the HRA, a survey tool that assesses all of a beneficiary’s needs including medical and behavioral health, chronic conditions, disabilities, functional impairments, assistance in key activities of daily living, dementia, cognitive and mental status. The HRA is used as a basis to develop the beneficiary’s individual care plan and make sure the beneficiary is connected to the care and services they need.

Cal MediConnect plans are required to conduct the HRA within 45 days for beneficiaries that have high risk, and 90 days for low risk beneficiaries. DHCS and the Centers for Medicare and Medicaid Services (CMS) have been working with the Cal MediConnect plans to track completion rates. Today, DHCS is releasing the HRA Dashboard to show HRA completion rates. Updated data will be released quarterly.
