As a part of the Coordinated Care Initiative Beneficiary and Provider Outreach Plan, DHCS is developing a toolkit of educational materials. The first set of these materials is now available in draft form for stakeholder review. DHCS will be hosting a call on Friday, February 7th at 1 pm to solicit comments and questions about the materials before they are finalized and distributed.

- Power point presentations for beneficiaries, advocates and providers.
- Fact sheets on selected topics, including eligibility, rights & responsibilities, and continuity of care.
- General brochure.
The goal of these materials is to ensure beneficiaries understand their choices.
After incorporating stakeholder feedback, revised versions of these documents will be posted to this site for download by the public, as well as used in outreach and education presentations and other events. The toolkit materials will continue to be updated as needed, and additional materials will continue to be developed as needs are identified.
