The second meeting on May 17, 2012 focused on how care coordination teams that would help beneficiaries with their care in the duals demonstration would work with the IHSS program.
There were two panel discussions. The first focused on how the care coordination currently occurs between the health plan and IHSS program in San Mateo County and identified opportunities and barriers. The second panel discussion focused on the intake and assessment processes.
